Park Place of Elmhurst, a not-for-profit five-star CCRC living community, is located across from Endeavor Health Elmhurst Hospital. Our 16-acre arboretum-designated campus offers 180+ customizable 1-, 2-, and 3-bedroom independent living apartments and a full continuum of care, including assisted living, short-term rehabilitation, skilled nursing, and memory support. We are seeking a Full-Time, Registered Nurse (RN) for Campus Care to support carrying out our mission to deliver quality care to our residents!

Key Responsibilities:
The primary purpose of your job position is to provide and/or coordinate campus wide home health, hospice and private duty services within an assigned Senior Living Community. As the Campus Care Nurse, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
  • Performs all functions as stated in the definition of "Registered Nurse" in the state board of health regulations.
  • Performs all functions in compliance with CDC, OSHA, JCAHO, State, and Federal Regulatory Bodies.
  • Serves as the primary care nurse for all assigned cases and provides skilled nursing services.
  • Teaches, supports and supervises client and caregivers, auxiliary nursing personnel, home health aides and others providing home health care.
  • Develops a client plan of care utilizing the written physician's orders, which prioritizes and includes all aspects of the physical, financial, and emotional resources of the client.
  • Communicates and interprets the goals of care to the client and caregiver with client authorization.
  • Completes written documentation for the medical record of all observations and actions according to PAH Policy and Procedures.
  • Facilitates coordination of follow-up of client care needs after visit, including notifying physicians of lab results and client status, follow through on new physician orders and revising plan of care/scheduling as necessary.
  • Participates in client care conferences.
  • Participates in the on-call system as assigned.
  • Shares in the responsibility with the client and caregivers in procedures designed to prevent and control the spread of communicable diseases and applies measures for prevention and control as required by local health regulations.
  • Participates in required in-service training programs and orientation of new staff as requested by clinical Supervisor.
  • Participates on committees designed to further the goals, philosophy and mission of the agency.
  • Provides monthly health education to Independent Living residents
  • Completes quality review of Campus Care Clients
  • Responsible for growth of the Campus Care program and business

Requirements
  • Graduated from a state accredited nursing program.
  • Current license to practice professional nursing in the state of practice.
  • Previous experience in public or home health nursing, minimum of 3 months
  • Minimum 1 year of medical/surgical experience
  • Bachelor's Degree in Nursing with 6 months experience in nursing.
  • Must demonstrate the culture of the company with professionalism and competence.
