Role Summary

Are you a compassionate, skilled Registered Nurse who thrives on providing high-quality, personalized care to patients in the comfort of their homes? We are seeking a dedicated Registered Nurse for a full-time position providing home health and hospice services.

In this vital clinical role, you will lead and coordinate an interdisciplinary team to deliver comprehensive, dignified physical, emotional, and psychosocial care to patients and their families. You will manage patient care plans, perform routine clinical assessments, educate caregivers, and serve as the primary liaison between physicians, patients, and the broader care team. If you excel at independent fieldwork, pain and symptom management, and building meaningful connections with patients and families, we invite you to apply.

Duties & Responsibilities:
  • Comprehensive Assessments: Perform initial and ongoing clinical assessments for home health and hospice patients, developing and updating individualized plans of care in accordance with physician orders.

  • Direct Care & Symptom Management: Deliver skilled direct nursing interventions, maintaining high proficiency in pain and symptom management during advanced illness or end-of-life care.

  • Interdisciplinary Leadership: Manage and coordinate care alongside Licensed Practical/Vocational Nurses (LPN/LVNs), Certified Nursing Assistants (CNAs), and support staff to ensure holistic patient and family care.

  • Caregiver Education: Instruct patients, family members, and caregivers on care plan execution, including medication administration, wound dressings, oxygen safety, and disease management.

  • Physician Collaboration: Act as the primary liaison connecting physicians, patients, and the care team, promptly reporting any changes in patient condition to attending and program physicians.

  • Clinical Documentation: Maintain accurate, timely, and complete electronic medical records, including assessment data, visit notes, orders, and care plans.

  • Team Supervision: Conduct routine supervisory visits of care team members at least monthly to ensure compliance with standards of practice.

  • Record Auditing & QAPI: Perform continuous record reviews to ensure regulatory compliance, quality service delivery, and active participation in Quality Assessment and Performance Improvement (QAPI) initiatives.

  • Safety & Infection Control: Adhere strictly to safety precautions, infection control protocols, standard precautions, and safety-sensitive position guidelines.

  • Other duties assigned.

Required QualificationsEducation, Licensure & Credentials
  • Licensure: Active, unencumbered Registered Nurse (RN) License in the state of practice.

  • Compliance: Ability to pass mandatory background checks, health screenings, and annual compliance training requirements.

Experience & Skills
  • Field Logistics: Must possess a valid driver's license, reliable personal transportation, and willingness to travel within an established 60-mile radius from the branch.

  • Clinical Capabilities: Strong background in clinical assessment, patient education, infection control, and case management workflows.

  • Communication & Resilience: Outstanding interpersonal, organizational, and written communication skills with the ability to manage patient caseloads independently.

Desired Qualifications
  • Home Health EHR: Prior experience using HomeCare HomeBase (HCHB) software is strongly preferred.

  • Specialized Care Background: Hands-on experience in Medicare home health, hospice, palliative care, or geriatric case management.

  • Pain Management Expertise: In-depth knowledge of pain and symptom control strategies for terminally ill patients.

  • Schedule: Monday through Friday, 8:00 AM – 5:00 PM (handling approximately 6–8 patient visits per day). Participation in an agency on-call rotation may be required.
