This is a fully remote position, allowing for flexibility while meeting the needs of our patients and their families. Our Hospice Center utilizes advanced telehealth technologies to connect with clients throughout the Spokane area, ensuring that our team can provide timely and effective support. You will collaborate with a multidisciplinary team, including medical professionals and other social workers, fostering a cohesive approach to patient care.

• Collaborate with healthcare team members to develop comprehensive treatment plans based on psychosocial assessments and established social work methodologies.
• Conduct thorough assessments of patients' needs, ensuring timely interventions that support emotional, social, and behavioral health.
• Maintain meticulous and timely documentation, including clinical notes and patient progress, in adherence to departmental policies and regulatory standards.
• Provide counseling and crisis intervention, actively exploring and addressing patients' emotional and behavioral challenges.
• Engage with patients’ families and support systems to discuss treatment plans and facilitate effective discharge planning from hospice services.
• Utilize electronic medical records and other relevant software systems to document and track patient interactions and care outcomes efficiently.

In this role, you will primarily work with patients facing terminal illnesses and their families, providing crucial psychosocial support during a vulnerable time. Your interventions will focus on enhancing quality of life, addressing emotional distress, and facilitating end-of-life planning. Your empathetic approach will be instrumental in helping families navigate complex emotions and decisions.

• A Master’s degree in Social Work from an accredited program is mandatory.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State is required.
• A minimum of two years of clinical social work experience is essential, particularly within healthcare or hospice settings.
• Proficient in computer systems including word processing, email communication, spreadsheets, and electronic medical records.
• Demonstrated ability to manage competing priorities within a fast-paced environment effectively.

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with telehealth platforms and electronic documentation practices.
• Advanced crisis intervention skills and a strong understanding of ethical practices in social work.
