• Conduct thorough psychosocial assessments to identify patients' mental health status, strengths, and needs, utilizing standardized assessment tools and personal interviews.
• Implement and facilitate evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), customized to address individual patient situations.
• Collaborate with a multidisciplinary team, including psychiatrists, nurses, and clinical staff, to ensure coordinated care that aligns with the patient's best interests and treatment goals.
• Educate patients and their families about diagnoses, treatment options, and potential medications, empowering them to make informed decisions about their care.
• Provide individual, group, and family therapy sessions, creating a safe space for expression, exploration of feelings, and coping strategies.
• Develop comprehensive treatment plans that reflect patient goals and preferences, updated regularly in collaboration with the care team.
• Assist patients in navigating the complexities of the mental health system, advocating for their rights and needs with community resources and agencies.
• Lead therapeutic group sessions, psychoeducation workshops, and discharge planning meetings to ensure continuity of care.

In this role, you will primarily serve patients and families dealing with terminal illnesses, ensuring they access necessary mental health services during their hospice journey. The focus is on enhancing quality of life, providing emotional and psychological support, and addressing grief and loss in a respectful and empathetic manner.

• Master's degree in Social Work from an accredited institution.
• Current active state licensure as a Licensed Clinical Social Worker (LCSW).
• Comprehensive knowledge of mental health disorders, treatment modalities, crisis intervention techniques, and the social determinants of health.
• Proficiency in working collaboratively within a multidisciplinary team to provide patient-centered care.

• Prior experience in hospice or palliative care settings is a significant advantage.
• Experience utilizing electronic health records (EHR) systems to document patient interactions and treatment outcomes.
• Familiarity with community resources and support services available to patients and families.
