• Conduct comprehensive behavioral health psychosocial assessments to identify clients' needs, strengths, and challenges.
• Implement evidence-based clinical interventions, providing individual, group, and family therapy to enhance coping strategies and emotional adjustment.
• Collaborate actively with the interdisciplinary treatment team by contributing insights from social work perspectives to optimize patient care plans.
• Research and channel community resources to aid patients and their families, including assistance programs for food security, healthcare, and housing.
• Provide crisis intervention for urgent situations, ensuring safety and well-being for high-risk clients, including those facing abuse or severe mental health crises.
• Monitor client progress regularly through follow-up appointments and adjustments to care plans as needed to ensure effective treatment outcomes.

Your primary focus will be on hospice patients and their families, who often experience significant psychological stressors related to serious illness, grief, and anticipated loss. Understanding the unique emotional landscape facing these patients is essential, and your expertise will guide them through navigational difficulties, ensuring that each individual receives tailored support that respects their preferences and circumstances.

• A Master's Degree in Social Work from an accredited program, with current licensure as a Clinical Social Worker (LCSW) in Pennsylvania.
• A minimum of two years of experience in a behavioral health setting, preferably with exposure to hospice care or end-of-life issues.
• Demonstrated experience with crisis intervention techniques and familiarity with psychosocial assessment tools.
• Skills in conducting comprehensive case management evaluations, including suicide risk assessments and forensic history-taking techniques.
• Understanding of and ability to access community resources that can assist clients in enhancing their quality of life during their treatment.

• Experience working with diverse patient populations, including veterans, to address their specific needs and challenges.
• Knowledge and experience in providing family therapy and addressing systemic issues surrounding a patient’s care.
• Proficiency in telehealth services and technologies for delivering remote therapy and assessments.
