You will collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and other allied health staff. • Conduct thorough clinical assessments to evaluate the social, emotional, and psychological needs of patients and families.
• Develop, implement, and evaluate individualized treatment plans based on evidence-based practices.
• Utilize solution-focused and strength-based approaches to therapy, addressing a variety of needs including grief, loss, and adjustment issues.
• Deliver therapeutic interventions that incorporate the latest research and best practices in mental health care.
• Document all interactions and treatment progress in clinical records, ensuring compliance with legal and ethical standards.
• Collaborate with the multidisciplinary team to enhance care delivery and address patient priorities effectively.
• Provide education and resources to patients and their families, empowering them to navigate their health journeys.
• Participate in ongoing training and professional development to stay current with advancements in social work practice and hospice care.

In this role, you will provide essential support to patients in hospice care who are often facing terminal illnesses. Your expertise will help families cope with the emotional complexities related to grief, loss, and end-of-life planning. You will engage with a diverse patient demographic, ensuring respectful and culturally informed practices that cater to individual needs. • A Master's Degree in Social Work from an accredited institution is required.
• A valid LCSW license to practice in Pennsylvania.
• Deep understanding of solution-focused therapy techniques and experience working with terminally ill patients preferred.
• Exceptional communication and interpersonal skills, allowing you to connect meaningfully with patients and their families.
• Strong problem-solving abilities and emotional resilience.

Experience in hospice care or palliative settings is a plus, providing insight into the unique challenges patients and families face. Familiarity with electronic health record (EHR) systems will be beneficial for maintaining accurate patient files and treatment documentation.

The salary for this full-time position ranges from USD 80,000 to 100,000 per year, commensurate with experience.
