• Conduct comprehensive clinical assessments to identify patient needs and develop effective individualized treatment plans, utilizing solution-based and evidence-based practices.

• Implement therapeutic interventions tailored to meet the unique mental health needs of clients, ensuring strategies are aligned with best practices.

• Maintain accurate and timely documentation in compliance with state regulations and internal policies, ensuring that clinical records reflect all interactions and interventions.

• Engage collaboratively with a multidisciplinary team, including physicians, nurses, and other social workers, to ensure coordinated and holistic patient care.

• Participate in regular case consultations and peer review meetings, offering and receiving feedback that enhances the quality of care provided.

• Educate patients and their families about mental health resources and coping strategies, empowering them to effectively manage their situations.

• Stay abreast of current research and developments in social work and mental health, applying new knowledge to enhance patient outcomes and organizational practices.

In this role, you will work primarily with individuals and families coping with terminal illnesses, providing psychological support during one of the most challenging periods of their lives. You will offer compassionate care that focuses on emotional well-being, grief support, and the overall quality of life for both patients and their loved ones.

• Must possess a Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW-C license in the state of Maryland is required.

• Minimum of 2 years of clinical experience, specifically in mental health or hospice care.

• Proficiency in delivering solution-based psychotherapy interventions; experience with end-of-life care is a plus.

• Strong communication, empathy, and problem-solving skills are essential.

• Ability to work effectively in a collaborative and team-oriented environment.

• Experience with interdisciplinary care teams, particularly in hospice or palliative care settings.

• Prior experience in conducting therapy sessions via telehealth platforms or remote modalities.

• Demonstrated ability to manage a high caseload effectively while maintaining compliance and quality standards.
