• Conduct thorough assessments and provide direct therapeutic interventions to clients presenting with mental health issues, ensuring the application of evidence-based practices.
• Develop and execute integrated care plans that facilitate seamless access to critical services, coordinating with clients, family members, and multidisciplinary teams.
• Engage in routine client meetings regarding assessments, ongoing care, and discharge plans, ensuring that therapeutic services align with best practices and adherence to ethical standards.
• Provide crisis intervention as necessary, demonstrating quick thinking and effective response strategies to mitigate client distress.
• Regularly participate in team meetings, contributing to collaborative discussions, and enhancing team-based approaches in service delivery.
• Manage documentation and reporting duties to comply with organizational standards and regulatory requirements, submitting accurate information in a timely manner.
• Stay informed about community resources and refer clients accordingly to enhance their support network.
• Work on continuity of care by following-up with clients during scheduled outreach and communication efforts.

As a Licensed Clinical Social Worker, you will primarily serve clients in Hartford, Connecticut, who are from diverse cultural backgrounds and often experience inequities in access to mental health services. Our client base includes children, adolescents, and families who encounter various challenges, including but not limited to trauma, anxiety, depression, and behavioral issues. Your role will directly influence their mental health journeys by providing compassionate care tailored to their unique situations.

• Must hold a valid Licensed Clinical Social Worker (LCSW) license.
• A Bachelor’s Degree in Social Work or a related field is required.
• A minimum of two years of experience in a clinical or community service setting is necessary.
• Demonstrated understanding of mental health issues, treatment planning, and therapeutic modalities.

• Experience working with underserved populations and complex cases.
• Familiarity with integrated care models and collaborative practice.
• Proficiency in data management systems used for clinical documentation.
• Knowledge of mental health regulations and ethical standards.

This position offers a competitive annual salary range of USD 80,000 to 100,000, commensurate with experience and qualifications.
