Overview

As a Psychiatric Social Worker (PSW) the incumbent will coordinate the development, implementation and monitoring of the patient's plan of care throughout the continuum by collaborating with the Attending Physician, the treatment team and the patient/patient's representative. In addition, the PSW identifies and arranges for discharge planning needs in a timely manner.

*Shift Hours - 8:00AM - 4:30 PM
*Days of Week - Monday- Friday

Responsibilities

The Psychiatric Social Worker (PSW) is responsible for the initial and ongoing assessment of assigned patients to ensure that services provided are medically necessary and provided in the most appropriate setting. 


Qualifications

Age of Patients Served: Adult (18-64 years) Geriatric (=>65 years)

Required Education: Bachelor's in Nursing, -OR- Masters in Social Work or related field. New hire RNs, effective June 14, 2017, must have a BSN degree from an accredited school of nursing within three (3) years of hire or transfer into the position. New hires or transfers into in the position between August 1, 2013, and June 13, 2017, must abide with the conditions in his or her offer letter and the verbiage in the job description signed upon hire or transfer into the position. Current employees who were hired or transferred into the position prior to August 1, 2013, will be grandfathered.

Required Experience: 1-2 years - Discharge planning and/or utilization review experience in a health care or managed care environment. Preferred Experience: 1-2 years' experience in a psychiatric hospital setting. 

Required Licensure/Certifications: -Licensed to practice nursing in the state of Florida -Masters in Social work (or related field) accepted in lieu of RN licensure.

Preferred Education: BSN preferred; certification in Case Management (CCM) preferred; MSW preferred. Necessary Skills: Excellent oral and written communication skills. Knowledge of community agencies and resources. Effective organizational skills. Computer experience (i.e., Word, Excel, Email).
