POSITION: LIVING WITH PURPOSE COMMUNITY PROJECT LIAISON

CLASSIFICATION CODE: EXEMPT FULL-TIME

DEPARTMENT: LIVING WITH PURPOSE

REPORTS TO: LIVING WITH PURPOSE PROGRAM MANAGER

STATEMENT OF JOB: Community Project Liaison works closely with the other Career Coaches, Case Managers, Therapists and Program Manager in the Living With Purpose program to provide responsive, high-quality services and programs in the White Mountain Apache Tribe under the authority of Apache Behavioral Health Services. The Community Project Liaison for the Living With Purpose program provides employment support services oversight, guidance, transportation. Community Project Liaison are responsible for assisting participants with navigating program services, needs-related resources connections and entry into employment in targeted sectors and other related services to support client independence and retention. The Community Project Liaison works closely with the outreach, partner development and services delivery teams to identify and match participants with opportunities that result in milestone achievements for the program and its participants. Community Project Liaison functions may include providing individual and targeted assistance with professional development and preparation for employment, identifying and connecting participants with education, skills training, work-based training, and placement opportunities, for work projects. provide guidelines for the community project. Provides timelines for completion of projects, ensures understanding of assessment criteria and ensures projects meet deadlines.

DUTIES AND RESPONSIBILITIES: 

• Manage project delivery: respond to referrals and work orders, ensure the provision of good quality individual assessments and support planning involving the service user and carer and maintain a high quality of service for service users and carers.
• Participate in internal and external assessments and reviews: representing the organization at meetings as appropriate.
• Transport clients to appropriate services and work project sits as needed.
• Provide information: raise public awareness about the service, also other support agencies and give advice and information about services. Facilitate access to other agencies/resources.
• Participant Support: train, support, supervise, appraise, and develop staff and volunteers. Project workers will also assist with the recruitment and Support Workers
• Service Planning: to provide information and reports regarding service activity to contribute to the service plan.
• Work with Carers: in line with local arrangements develop and maintain a range of carer support and education tailored to the needs of the area.
• Help participants identify habits that prevent consistent employment.
• Attend and contribute to meetings: feedback to staff policy and operational changes and issues discussed.
• Works as community outreach and engagement, particularly with disadvantaged groups.
• Ability to work with a variety of stakeholders, including residents, local groups, and organizations across the voluntary and public sectors.
• Ability to support and mentor participants within a community project.
• Hard-working, reliable, and professional, able to manage a variety of challenging workloads and work well both as part of a team and alone.
• Willing and able to work in different environments, including being office based and spending a large amount of time on the grounds within communities.
• Coordinate with Living With Purpose Career Coaches, Case Managers, Therapists and Program Manager to identify and refer high risk individuals to appropriate support services and community resources.
• Work with Living With Purpose Career Coaches, Case Managers, Therapists and Program Manager to assist with the coordination, scheduling, outreach, and promotion of the Living With Purpose work project offerings.
• Oversee work project sites to monitor attendance, ensure a physically and emotionally safe work environment and timely project completion.
• Maintain inventory of supplies and equipment. Submit requests for order or purchase of necessary supplies or equipment to Program Manager.
• Maintain accurate records of work orders, develop reports and keeps up to date accurate documents as requested using CMMS.

MINIMUM QUALIFICATIONS:

• Must be at least 21 years of age.
• Must possess a high school diploma or G.E.D.
• Must have a valid driver’s license, Motor Vehicle Clearance, and reliable transportation.
• Ability to pass a level one fingerprint clearance card.
• Effective written and oral communication skills, as well as basic supervision skills. 
• MS Office proficiency.
• Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, and supervisors.
• Must demonstrate basic critical thinking, problem-solving, de-escalation, organizational and time management skills.

Preferred:

• At least 1 year experience working with Native Americans.
