Job description

Bilingual Intake Care Manager (Haitian Creole or Portuguese)
The Intake Care Manager, within the framework of the Executive Office of Aging and Independence (AGE) and Mystic Valley Elder Services (MVES) provides a consumer centered approach assessment, and service acquisition to determine program eligibility and to assist older adults and those with disabilities to live independently in the community.
Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer a hybrid work schedule (once training is complete.) No nights or weekends.
Compensation: Starting annual salary range is $54,566.15 - $58,783.24. A 6% increase will be used to recognize the required bilingual skill set in Haitian Creole or Portuguese that is required for this role.

Job Activities
Job activities of the position are below. Additional duties may be assigned as required.
Top Key Activities:
  • Conduct assessments of consumers by telephone, video conferencing, and by visiting the consumer in their home to assess the older adult's history, current status, living situation, and support system, and if applicable, caregiver's history, and identify unmet needs, which inhibit independent living, utilizing a person-centered approach for eligibility of programs.
  • Initiate services when immediate intervention is required.
  • Provide information and referral to ineligible applicants.
  • Develop, authorize, and implement an initial care plan within budget guidelines to meet identified needs.
  • Coordinate community-based services, and ensure access to public benefits with consumers, families, caregivers, and service providers.
  • Provide crisis case management prevention and intervention.
  • Document and maintain accurate consumer information in case records and client services database in accordance with Executive Office of Aging and Independence (AGE) and Mystic Valley Elder Services(MVES) standards and timelines.
  • Perform other appropriate duties as assigned by supervisor.

Other Activities:
  • Participate in interdisciplinary/collaborative model of case management.
  • Participate in the orientation and training process for new staff.
  • Develop professionally through training opportunities.
  • Attend and participate in supervisions and all staff meetings as required by manager.

  • Successful conversion of referrals to home care eligible consumers

Qualifications:
  • A Bachelors' degree preferred from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least two years of experience in counseling, casework or case management.
  • Fluent in English and Haitian Creole or Portuguese.
  • Excellent interpersonal skills with demonstrated ability to communicate (written and verbal) across all levels, including staff, family and external partners.
  • Strong interviewing, assessment and organizational skills.
  • Ability to maintain confidentiality while working with sensitive information or in sensitive situations.
  • Highly resourceful team player, with the ability to work effectively independently or as a team.
  • Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved.
  • Private transportation required.
  • Proficient in the following platforms: Microsoft Office (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications.

Physical Requirements of the Job:
  • Ability to lift, push, pull and carry objects weighing up to 25 pounds
  • Ability to climb up and down stairs unassisted
  • Ability to travel into the community
  • Work requires regular standing, stooping, and bending
