Job description

Coverage Needed

  • Start Date: As soon as credentialed
  • End Date: Ongoing for 2-3 months
  • Schedule: Full time
  • Cover 2 sites

Clinical Details

  • Average clinic volume: 22 patients per day
  • Average inpatient volume: 15 - 20
  • High volumes
  • Special Procedures to be performed: Permanent Pacemaker implants, Pacemaker Lead extractions, Leadless Pacemaker implants, ICD implants, Ablations (Atrial Fibrillation, Atrial Flutter, SVT, VT Cryoablation)
  • Support Staff: 2 cardiac hospitalists

Requirements

  • Active IN License
  • DEA (or one that is easily transferable) and CSR
  • Board Certified
  • EPIC experience preferred, but willing to train
  • CV must be in Month / Year format
  • Must have license in hand at time of submission, or proof that the license application has been submitted
  • Any gaps greater than 30 days will require an explanation on the CV
  • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
