Under the supervision of a RN and in accordance with agency policies and state practice, the Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.
Responsibilities
  • Performs prescribed individualized patient care to patients as delegated.
  • Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team.
  • Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
  • Observes, documents and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.

  • Qualifications
    REQUIRED QUALIFICATIONS
    • Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold a compact license.
    • Graduate of an accredited school as a Licensed Practical Nurse

    PREFERRED QUALIFICATIONS
    • One year of experience as an LPN in a home health care setting is preferred

    Required Licensure/Certifications
    - Unencumbered LPN license in VT and NH or compact.
    • Area of Interest: Nursing
    • Pay Range: $29.95/Hr. - $46.42/Hr.
    • FTE/Hours per pay period: 1.00 - 1.00 - 40 hrs/week
