Physical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

The PT will:

Β·       Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.

Β·       Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.

  • Communicate with all members of the health care team.
  • Document evaluations, treatment goals, and objectives.
  • Teach patients and family members the rationale for treatments and procedures.

REQUIRED QUALIFICATIONS

  • Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school in Physical Therapy

PREFERRED QUALIFICATIONS

  • One year of experience as a PT in a home health care setting is preferred

  • Must hold at least one active unencumbered PT license in either VT or NH at the time of hire. Employees must have active unencumbered PT licenses in both VT and NH within 30 days of their hire date.
  • Valid driver’s license with a clean driving record
  • Current car insurance, which meets minimum standards
  • BLS within 90 days of hire
