Senior Front-End & FHIR Interoperability Engineer

About the Opportunity

We are building a new engineering team to develop and scale a modern, cloud-native healthcare technology platform. The work sits at the intersection of clinical software and healthcare interoperability.

This is an early-stage opportunity to shape a high-impact product from the ground up, backed by an experienced leadership team with deep healthcare-technology expertise. You will help set the technical foundations the platform is built on and see your work reach real users in clinical settings.

Position Summary

We are seeking a Senior Front-End & FHIR Interoperability Engineer to build and own a healthcare application end-to-end. This is a hands-on builder role for someone who is first and foremost a strong front-end engineer, and who has real, practical experience integrating with EHR systems and building SMART on FHIR applications.

You will write the production front-end code, build and maintain the application, implement the EHR integrations and SMART on FHIR launch, and own the mapping between clinical data and the user interface. You should be equally comfortable reasoning about interoperability constraints and shipping polished production front-end code.

What Youโ€™ll Own
  • The front-end application end-to-end โ€” you build it, and you maintain it.
  • The integration between EHR systems and the application: EHR-embedded launch, secure authentication (OAuth 2.0 with PKCE), and session handling.
  • The mapping layer: translating clinical data from EHR/FHIR sources into the internal model and the fields the UI needs. This is where interoperability meets the front end, and it is the heart of the role.
  • Reusable integration patterns that let the platform scale to additional EHR connections over time.
Key Responsibilities
  • Build and maintain the complete front-end application and every workflow screen โ€” from first build through ongoing iteration and production support.
  • Build and maintain SMART on FHIR application launch and consume FHIR R4 resources under a read-only constraint, aligned to USCDI v3.
  • Implement CDS Hooks integrations and drug-in-context triggering.
  • Design and own the data mapping between EHR/FHIR resources and the application model and UI.
  • Build clean, fast front-end views that surface mapped clinical data and keep the coordinator work queue legible under load.
  • Contribute production-quality code and participate in design and code reviews with the engineering team.
  • Partner with Product, QA, and the engineering team to deliver secure, reliable, maintainable software.
  • Build to the platform guardrails.
Technology Environment

  • Interoperability: FHIR R4, SMART on FHIR, CDS Hooks, USCDI v3, OAuth 2.0 with PKCE
  • Backend: Python, FastAPI, REST APIs
  • Database: PostgreSQL
  • Cloud: Microsoft Azure
  • DevOps: Azure DevOps, CI/CD pipelines
Required Qualifications
  • 6+ years of software engineering experience with a strong track record building and shipping production front-end applications in React and TypeScript.
  • Hands-on experience integrating with EHR / EMR systems.
  • Has built and shipped at least one production SMART on FHIR application.
  • Working knowledge of FHIR R4 and OAuth 2.0 / PKCE.
  • Hands-on CDS Hooks implementation experience.
  • A practical mapping instinct โ€” comfort translating messy, real-world clinical data into clean models and clean UI.
  • Familiarity working with PHI in a HIPAA environment.
  • Excellent communication and the discipline to work precisely on a lean team.
Preferred Qualifications
  • Experience with specific EHR vendors (e.g., Epic, Cerner / Oracle Health, athenahealth, or smaller ambulatory EHRs), including sandbox onboarding.
  • USCDI v3 depth.
  • Healthcare access, pharmacy, or payer-integration domain knowledge.
  • Exposure to eligibility (270/271) and real-time benefit workflows.
  • Working familiarity with Python/FastAPI and the Azure ecosystem.

  • Comfortable switching between deep interoperability work and hands-on UI work in the same week.

Pay Range: Actual compensation will be determined based on the candidate's experience, qualifications, and work location. Candidates in different geographic markets may be offered compensation that reflects local market conditions.
