Physical Therapist (PT) Home Health - Full-Time
Location: Austin, Round Rock, Georgetown, Pflugerville, Manor, Hutto, Leander, Florence, Jarrell, TX
TheraCare is seeking a licensed Physical Therapist (PT) for a Full-Time home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Austin, TX and surrounding areas.
This Full-Time home health PT position is ideal for clinicians looking for flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting. Therapists enjoy creating their own schedules while helping patients improve mobility, strength, safety, and overall independence in the comfort of their homes.
Job Responsibilities
  • Perform home health physical therapy evaluations and re-evaluations, including OASIS assessments, for adult and geriatric patients
  • Develop, implement, and update individualized Plans of Care (POC) based on patient goals, diagnoses, and physician orders
  • Provide skilled physical therapy treatment for orthopedic, neurological, cardiac, post-surgical, and medically complex conditions
  • Deliver evidence-based interventions focused on mobility, gait training, balance, strengthening, fall prevention, pain management, and functional independence
  • Apply appropriate therapeutic techniques and physician-ordered modalities, including manual therapy, heat, cold, and electrical stimulation
  • Educate patients and caregivers on home exercise programs, safety awareness, transfer training, fall prevention, and use of assistive and durable medical equipment
  • Monitor patient progress toward functional goals and modify treatment plans as clinically indicated
  • Complete accurate, timely, and compliant documentation within the EMR at the point of care
  • Communicate and collaborate effectively with physicians, nurses, case managers, PTAs, home health aides, and interdisciplinary care teams
  • Supervise Physical Therapist Assistants (PTAs) and home health aides in accordance with state regulations and agency policies
  • Participate in case conferences, discharge planning, care coordination, and ongoing interdisciplinary communication
  • Ensure compliance with home health regulations, Medicare guidelines, infection control standards, and patient safety protocols
  • Maintain professional competency through ongoing education, training, and adherence to evidence-based practice standards
  • Perform additional duties as assigned to support quality patient care and operational excellence
