The Physical Therapist position is primarily based onsite at our rehabilitation facility in Sayreville, NJ. • Evaluate and treat patients in a home care environment following injuries, surgeries, or medical conditions.
• Develop individualized treatment plans using a variety of therapeutic modalities, including exercises, stretching, hands-on treatments, and the use of equipment to alleviate pain and increase mobility.
• Document patient progress, treatment responses, and modify therapy programs as necessary.
• Reassess therapeutic goals and update treatment plans to meet the evolving needs of the patient.
• Guide patients and their families in using assistive devices effectively, enhancing self-care and ambulation.
• Provide training to caregivers and other health team members regarding rehabilitation procedures and the use of assistive modalities.
• Continuously enhance your professional knowledge through workshops, publications, and participation in relevant professional organizations.

Our patients span a wide range of diagnoses, from post-operative rehabilitation to chronic pain management, encompassing all age groups. As a Physical Therapist, you will work closely with these patients, adapting your treatment approaches to each individual's specific health challenges and goals. • A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current state license in Physical Therapy to practice in New Jersey.
• Strong verbal and written communication abilities to effectively interact with patients and team members.
• Proficiency in word processing and basic computer applications.
• Agility and flexibility to adapt to the diverse needs of patients and workflow adjustments.

• Minimum of 2 years of experience as a practicing Physical Therapist is highly preferred.
• Experience in home healthcare or rehabilitation settings is an advantage.
• Familiarity with current best practices in physical therapy and rehabilitation science.
