JOB SUMMARY:
The Regional Nurse Consultant, RN provides clinical leadership, operational support, regulatory oversight, and quality improvement guidance to assigned skilled nursing facilities. Working closely with Directors of Nursing Services (DNS), Administrators, MDS teams, and facility leadership, the Regional Nurse Consultant supports the delivery of high-quality resident care while ensuring compliance with federal and state regulations, company standards, and reimbursement requirements.
This position serves as a clinical resource and mentor to facility leadership teams and may provide interim Director of Nursing Services coverage during vacancies, leaves of absence, or other operational needs.
Frequent travel throughout Oregon is required.
RESPONSIBILITIES:
  • Provide clinical consultation, leadership, and support to Directors of Nursing Services, MDS Coordinators, Infection Preventionists, and facility leadership teams
  • Monitor resident care delivery systems to ensure compliance with company standards and regulatory requirements
  • Evaluate clinical outcomes, quality measures, and operational performance indicators and assist facilities in achieving established goals
  • Assist with development and implementation of action plans to address identified clinical concerns and opportunities for improvement
  • Promote evidence-based practices and resident-centered care throughout assigned facilities
  • Support facilities in maintaining compliance with federal, state, and local regulations governing skilled nursing facilities
  • Conduct mock surveys, clinical audits, and compliance reviews
  • Assist facilities with survey preparation, survey management, and plans of correction
  • Review clinical documentation and systems to ensure survey readiness and regulatory compliance
  • Provide guidance regarding CMS regulations, state survey requirements, F-tags, and quality initiatives
  • Support facility QAPI programs and initiatives
  • Analyze quality measures, Five-Star ratings, infection control metrics, falls, rehospitalizations, medication management, and other clinical indicators
  • Assist facility leadership with developing corrective actions and monitoring outcomes
  • Identify trends and opportunities to improve resident outcomes and operational effectiveness
  • Partner with MDS teams and facility leadership to support accurate assessments, documentation, care planning, and reimbursement processes
  • Monitor systems impacting Medicare, Medicaid, and PDPM reimbursement
  • Assist facilities in improving clinical documentation practices to support quality care and reimbursement accuracy
  • Collaborate with interdisciplinary teams to ensure regulatory and reimbursement compliance
  • Serve as Interim Director of Nursing Services during vacancies, leaves of absence, or periods of operational need
  • Provide direct oversight of nursing operations, staffing, clinical programs, and regulatory compliance
  • Support recruitment, onboarding, training, and development of new Directors of Nursing Services
  • Assist with staffing stabilization efforts and implementation of operational improvement plans
  • Support facility Infection Prevention and Control Programs
  • Monitor infection control practices, outbreak response activities, and regulatory compliance
  • Participate in clinical investigations, resident incidents, grievance reviews, and risk management initiatives
  • Assist facilities in maintaining emergency preparedness plans and clinical response systems
  • Mentor and develop Directors of Nursing Services and other clinical leaders
  • Assist with leadership development, succession planning, and clinical education initiatives
  • Provide training related to regulatory compliance, clinical best practices, documentation, infection prevention, and quality improvement
  • Support onboarding and orientation of newly hired clinical leaders
  • Collaborate with Administrators, Regional Operations leadership, Medical Directors, Therapy Services, Pharmacy Consultants, and other healthcare professionals
  • Communicate facility performance, clinical concerns, and recommendations to organizational leadership
  • Promote a culture of accountability, teamwork, quality outcomes, and continuous improvement
  • Other duties as assigned

REQUIREMENTS:
  • Active Registered Nurse (RN) license in the state applicable
  • Minimum five (5) years of nursing leadership experience in skilled nursing or long-term care
  • Minimum three (3) years of experience as a Director of Nursing Services, Assistant Director of Nursing, Regional Nurse Consultant, or equivalent clinical leadership role
  • Strong working knowledge of CMS regulations, state survey processes, and skilled nursing reimbursement systems
  • Experience with QAPI programs, infection prevention, quality measures, and survey management
  • Valid driver's license and ability to travel extensively
  • Multi-facility clinical oversight experience
  • Prior experience as a Regional Nurse Consultant
  • Experience supporting PDPM reimbursement and MDS operations
  • Experience leading facilities through surveys and plans of correction
  • Infection Prevention certification or specialized clinical certifications
  • Extensive knowledge of skilled nursing regulations and standards of practice
  • Strong leadership, coaching, and team development skills
  • Ability to assess clinical operations and implement effective solutions
  • Strong analytical, organizational, and problem-solving abilities
  • Excellent communication and relationship-building skills
  • Ability to manage multiple facilities and priorities simultaneously
  • Proficiency with electronic medical records and clinical reporting systems
