Job description

Job Summary:

Position Summary: Responsible for providing professional nursing care to patients in a caring and compassionate manner following established standards and practices. Provides excellent customer service to patients and family members both in person and over the telephone while working collaboratively with the healthcare team.

Qualifications:

Education:

Two year college (Associate's Degree)

Years of Experience:

1 - 3 years (minimum of 2 yrs exp)

Licensure(s):

RN State of Maine License

Certification(s):

BLS/CPR, AED certification

Position Specific Competencies:

  • Supports and promotes the mission and values of Covenant Health Ministries
  • Assists with patient education
  • Screens and documents patient calls determining the urgency of the problem; consults with provider regarding treatment and advises patient accordingly
  • Performs patient triage
  • Documents charts including returned phone calls, missed appointments or other clinical issues, with initials, date and time.
  • Takes messages and assists providers with return phone calls
  • Calls in prescriptions per provider orders and protocols
  • Retrieves records and files/imports patient test results. Notifies patient of test results per provider direction thoroughly documenting patient outreach in a timely manner
  • Coordinates and completes paperwork/forms as directed related to referral, health, school, sports, immunizations, excused from/return to work/school, childcare and medication administration. Charts completion as required.
  • Performs duties per scheduled hours arriving/leaving on time and completing assigned responsibilities.
  • Demonstrates flexibility in assisting with support/coverage withing the practice network
  • Other duties as consistent with this role.
  • Prioritizes patient care by implementing physician orders and nursing procedures
  • Supports providers with MyChart messages/desktop
  • Advises patients according to established clinical protocols when physicians input is not required
  • Critical analysis of patient laboratory data to assist in clinical decision making through the monitory of laboratory, x-ray, ECG results and other pertinent medical data under physician guidance
  • Assist in the completion of referrals
  • Providing results of testing inclusive of patient education as indicated
  • Ensures that patients receive appropriate and adequate instruction regarding new diagnoses, new medications, as well as wellness recommendations
  • Conducts nurse visits for patients assigned to the team
  • Review and triage of media manager files
  • Editing medication changes by outside provider (outside of MA scope)
  • Review CC'ed charts and communicate with provider/patient as needed
  • Leads and participates in daily huddles, weekly pre-planning, and monthly team meetings with core members of the team
  • Collaborates with internal and external services to facilitate care transition in order to effectively meet the patient's needs
