About the Role
Transform bedside care with AI. As Circadia's on-site NP, you'll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions-reducing preventable readmissions. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.
 
This is a full-time, embedded NP role requiring Monday-Friday, on-site presence at our partner facility in Los Altos, CA. 
What You'll Do
  • Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
  • Tech bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
  • Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
  • Close the loop: Complete documentation/charts within the same day
  • High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
  • Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
  • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams
How Success is Measured (KPIs)
  • Visits: Target 30 patient visits/day
  • Engagement: 80% Time to Acknowledgement Rate: <24h
  • Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
  • Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.
Must-Have Qualifications
  • Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
  • Eligible for Medicare participation; company bills on your behalf.
  • 2+ years NP experience with complex adult populations; able to practice independently.
  • Valid CA driver's license; comfortable traveling locally.
  • Thrives in a dynamic, evolving startup environment.
  • English required
Nice-to-Haves
  • Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
  • Spanish is preferred.

As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $130,000 - $160,000. The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
