The Role
Transform home-based care. As Circadia's full-time field home health NP, you'll see patients directly in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions-reducing preventable hospitalizations. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting.
What You'll Do
  • Direct patient care: Conduct initial in-home assessments on patients; initiate/adjust plans of care; coordinate with referring and primary care providers.
  • Tech - bedside integration: Introduce the Circadia Device during in-home assessment and help set-up in a 2 minute process.
    • >
  • Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.

  • Close the loop: Complete documentation/charts within the same day
    • >
  • Home-based visit management: Efficiently complete 6+ in-home visits per day across designated home health patients in the SoCal/OC area, focused on new admissions

  • Quality & product feedback: Surface field insights to improve workflows and tech integration in a home health setting
    • >
  • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.

  • Regional flexibility: Cover sites within a 30 mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)
  • Visits: Target 6+ patient home visits/day
  • Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.

Must-Have Qualifications
  • Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.

  • Eligible for Medicare participation; company bills on your behalf.

  • 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.

  • Valid CA driver's license; reliable transportation for in-home visits across the SoCal/OC area.

  • Comfortable conducting face-to-face visits in a home environment, including with patients requiring complex care.
