Child & Family Support Services (CFSS) is accepting applications for Full-time Case Manager in Coconino County.

The Case Manager position is perfect for a determined, experienced behavioral health technician (BHT), social worker or behavioral health professional looking for an exciting and rewarding career opportunity working with young adults and their families to be the primary point of contact in the coordination of comprehensive healthcare. This is not a supervisory position. Majority of time is spent in homes and the community rather than in an office setting.

Case Manager Job Responsibilities:

  • Providing ongoing case management; coordinating behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs.

  • Collaborating with the family and team members to create support plans that accurately capture the strengths, needs and culture of the family.

  • Facilitating Child and Family Team meetings; establishing an agenda, setting and maintaining a strength based meeting environment, staying within timeframes, giving all team participants the opportunity to contribute, holding people accountable for their commitments and the ability to assist a team in developing a consensus for children and families.

  • Assisting young people and families in securing needed community resources and linkages to other services. Helping families to identify natural and informal supports.

  • Service delivery can be inclusive of skills training and family support.

  • Interacting professionally with other stakeholders: Division of Child Safety (DCS), Division of Developmental Disabilities (DDD), juvenile probation, Direct Service Providers (DSP), etc.

  • Performing other duties related to the position

This position requires afternoon, evening, and weekend work. Adaptability in scheduling, driving throughout the county and working within the homes and communities of the families we support is required.

Case Manager Education/Experience Needed:

  • High school diploma or GED.

  • Must be BHT with two (2) years of related behavioral health work experience, or a related bachelor’s degree (preferred but not required).

Case Manager Requirements:

  • Must be 21 years or older

  • Valid Arizona Driver's License with an acceptable 39 month driving record

  • Reliable transportation, including proof of Insurance

  • Candidates are required to submit a negative drug test

  • Obtain an Arizona DPS Level One Fingerprint Clearance Card

  • Complete Adult, Child, and Infant CPR and First Aid training
