We're looking for a compassionate, organized, and relationship-driven Homecare Services Coordinator to guide participants through Minnesota's Community First Services and Supports (CFSS) program.

About This Role:

The Homecare Services Coordinator (Advisor) is responsible for developing and managing a service delivery plan with participants based on their county assessment under the CFSS program. The service deliver plan is developed in partnership with the participant and fulfilled by either an Agency or Budget provider. The Advisor will manage the success of the plan through ongoing communication with the participant, service agencies, lead agency providers, and DHS. Serving as a resource for participants in their case load, they are expected to stay abreast of current policies and changes with the State of Minnesota, counties, MCO agencies, and service agencies. Consultation Advisors play an integral role in participantsโ€™ abilities to receive and maintain goods and services that they require.

Requirements:
  • Bachelor's degree in Human Services, Social Work, Psychology, Healthcare, Rehabilitation, or a related field

OR

  • One year of full-time experience providing direct services to individuals with disabilities or older adults

OR

  • Personal experience coordinating your own long-term support services

Preferred

  • Experience in case management, care coordination, social services, disability services, healthcare navigation, or aging services.
  • Knowledge of Minnesota HCBS, PCA, CFSS, waiver programs, or related services.
  • Experience managing budgets, service plans, or participant-centered programs.
  • Proficiency with Microsoft Office and electronic documentation systems.
