Position Overview

We are seeking a dedicated and skilled Physical Therapist to join our team at Cape Cod Physical Therapy in Highland Park, New Jersey. This full-time position focuses on delivering exceptional rehabilitation services to our patients in the comfort of their own homes, ensuring they have access to high-quality care tailored to their specific needs.

Work Environment

This position is primarily onsite as part of our home care service model, allowing you to work directly with patients in their home environments. Here, you'll employ a variety of therapeutic interventions to ensure that each patient receives personalized care tailored to their unique circumstances. You will have the flexibility to design your own work hours, typically Monday through Friday, allowing for a work-life balance that suits you and caters to the needs of your patients.

Key Responsibilities

As a Physical Therapist in our home care setting, you will:

  • Conduct thorough evaluations and assessments of patients recovering from injuries, surgeries, or managing chronic illnesses.
  • Develop individualized treatment plans that incorporate a variety of therapeutic modalities, including exercise, hands-on therapy, and use of assistive devices, all aimed at improving patient mobility and alleviating pain.
  • Implement treatment plans effectively, adjusting techniques as needed to ensure optimal therapeutic outcomes.
  • Track and document each patient's treatments, responses, and progress meticulously using Electronic Medical Record (EMR) software, particularly WebPT, to facilitate precise documentation and continuity of care.
  • Regularly reevaluate patient conditions and modify therapeutic goals and strategies based on progress.
  • Educate and train patients, families, and caregivers on exercises and assistive devices that promote independence and safety.
  • Continuously enhance your knowledge and skills through participation in workshops, professional publications, and networking opportunities within the physical therapy community.
Patient Population / Client Focus

Your efforts will directly impact a diverse client base, including children and adults across various diagnoses requiring personalized treatment approaches for recovery and rehabilitation. You will work with individuals who have experienced trauma, surgical interventions, and those managing ongoing health conditions, providing holistic and compassionate care.

Qualifications

The ideal candidate will possess the following qualifications:

  • A Doctorate in Physical Therapy (DPT) from an accredited institution.
  • An active Physical Therapist license in New Jersey, with a flawless recordโ€”no history of sanctions or board proceedings.
  • Significant proficiency in various manual therapy techniques and exercise program design.
  • Strong familiarity with EMR systems, particularly WebPT, for accurate documentation and patient management.
  • Committed to staying current with evolving physical therapy methodologies and research.
Preferred Experience

While the necessary qualifications are essential, we also highly value experience that includes:

  • A background in home health care or a similar setting, demonstrating your ability to adapt therapies to various environments.
  • Experience working with a wide range of patient populations, showcasing your versatility and understanding of individualized patient care.
