About the Role:
As a Social Worker, you will be responsible for:
Assessing the social, emotional, and psychological needs of our residents and identifying opportunities for improvement
Developing and implementing individualized care plans to ensure our residents receive comprehensive care
Providing counseling, guidance, and support to residents and their families
Collaborating with other healthcare professionals to ensure a coordinated approach to care
Identifying and addressing social determinants of health that impact our residents' overall well-being
Responsibilities:
Conduct initial and ongoing assessments of residents' social, emotional, and psychological needs
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Provide counseling, guidance, and support to residents and their families
Identify and address social determinants of health that impact our residents' overall well-being
Participate in family and resident meetings, as needed
Maintain accurate and detailed records of resident interactions and care plans
