Job Summary
We are seeking a Home CareRNto join our team. In this role, you will be the DON primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Responsibilities
  • Who has held a position of a supervisor in home health care for at least two yrs and able to prepare a comprehensive care plan.
  • Coordinate and supervise other colleagues to assess, implement and evaluate staff performance
  • Able to do admin work and also comply with accreditation process
  • Document any changes in patient conditions and able to do incident report documentation
  • Maintain accurate records of patient care and discuss observations with the management
Qualifications
  • Currently licensed Registered Nurse (RN) in state of employment
  • Previous experience as a Home Care RN with two YRS Experience would be preferred
  • Excellent in communication skills, interpersonal and client care skills
  • Valid drivers license ,Physicals, Xray, a copy of previous background checks will be a plus
  • First aid/CPR certification /malpractice insurance is required
