What You'll Do

  • Complete routine and PRN hospice visits throughout your assigned territory.
  • Perform comprehensive nursing assessments and ongoing symptom management.
  • Provide pain management, wound care, medication education, and comfort-focused interventions.
  • Partner with physicians, social workers, chaplains, hospice aides, and families to coordinate individualized plans of care.
  • Maintain accurate, timely documentation within the EMR.
