Job description

Job Summary:
RN is a member of the patient care team within the Department of Home Health and is directly accountable to the Patient Clinical Manager. The position requires that the individual be clinically competent and knowledgeable of home health regulations. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served, including pediatric patients; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The RN duties include assessments and reassessments of patients on an ongoing basis, initiating plans of care and making revisions as needed, demonstrating appropriate nursing skills and rehab and preventive nursing procedures. Coordinates patient care with other disciplines and acts as patient case manager and patient advocate. Performs nursing skills in the home to include wound, ostomy, medication administration and IV therapy. The RN prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. RN is accountable for her documentation and ordering of supplies according to policies and procedures. Notifies the provider or his agent of any changes and instructs patient and family in meeting needs. RN participates in in-service and competency programs as applicable. Performs other job duties as assigned.
Performance Expectations:
  • Demonstrate ability to perform wound care for patients using wound VAC and troubleshooting equipment as needed.
  • Demonstrate ability to oversee medication administration in the home setting by peripheral and IV pump administration.
  • Demonstrate ability to enter all patients charting in EMR documentation timely and meeting state and federal regulations

Qualifications:
Education/Skills
Graduate of a recognized non-online RN program required.
Work Experience:
Minimum one year nursing experience.
Certification/Licensure-DUE UPON HIRE
  • Licensed RN able to practice within the State of MS
  • Valid Drivers License
  • Proof of Automobile Insurance

Additional Certification/Licensure - Obtained based on required timeframe below
  • Basic Life Support
