Title: Registered Nurse - Home Health

Assignment Details:

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
Roles & Responsibilities:
- Assess patients in home health and/or hospice settings to identify their physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families about care routines, diet, medications, and home safety tailored to their specific needs.
- Supervise and provide clinical direction to Home Health Aides and assistive nursing staff to ensure quality service delivery.
- Perform pain assessments and utilize appropriate pain management techniques.
- Communicate effectively with interdisciplinary teams, patients, and families.
