Job Title: Registered Nurse (RN) – Floating Visit Nurse | Home Health

Schedule: Full-Time
Compensation: $75,000 to $110,000 annualized (per-visit model based on volume)
Location: Greater Twin Cities Metro Area (21-county service area)

To Stand Out

Email Elena Ehrlich at elena.ehrlich@hhcare.net with a summary of your RN experience, home health background, and why you're interested in a flexible field nursing opportunity.

Love Home Health, But Don't Want the Same Territory Every Day?

Some nurses enjoy the familiarity of a consistent territory.

Others enjoy the excitement of variety.

This role is designed for nurses who like experiencing different communities, supporting multiple clinical teams, and caring for a diverse patient population while maintaining the independence that makes home health nursing so rewarding.

Instead of being assigned to one permanent territory, you'll help wherever patient demand is highest, giving you the opportunity to broaden your clinical experience while making an immediate impact across our organization.

Why Nurses Choose This Opportunity

  • $75,000 to $110,000 annualized
  • Full-time schedule with consistent patient volume
  • Travel throughout the Twin Cities Metro with mileage reimbursement
  • Meaningful 1:1 patient care
  • Greater variety than a traditional home health territory
  • Collaborate with multiple experienced clinical teams
  • Expand your clinical knowledge by caring for a wide range of patients
  • Join a family-owned organization with more than 30 years of home health excellence

Why This Role Is Different

No two days are exactly alike.

One week you may support patients in the north metro. The next, you may spend time helping a team in another community.

That flexibility keeps your work interesting while allowing you to become familiar with different clinicians, referral sources, and patient populations throughout our service area.

For nurses who enjoy learning, adapting, and staying engaged, it's one of the most rewarding positions in home health.

What You'll Be Doing

  • Provide skilled RN home health visits throughout the Twin Cities service area
  • Complete comprehensive nursing assessments
  • Perform wound care, medication management, injections, and other skilled nursing services
  • Educate patients and caregivers on disease management and recovery
  • Collaborate with physicians, therapists, and interdisciplinary care teams
  • Support multiple territories based on patient needs and census
  • Complete accurate documentation using Homecare Homebase (HCHB)
  • Help ensure patients receive timely, uninterrupted care

Who We're Looking For

  • Active Minnesota Registered Nurse (RN) license
  • Experience in home health, hospital, rehabilitation, or skilled nursing preferred
  • Strong assessment and critical thinking skills
  • Comfortable working independently in a field-based environment
  • Flexible and adaptable to changing daily assignments
  • Reliable transportation and willingness to travel throughout the Twin Cities Metro
