Job description

Valley Health at Home by BAYADA is seeking a compassionate and dedicated Registered Nurse, RN to join our team and work with adult and senior clients on a per visit basis. This is a full time position.

At least one year of current or recent hands-on medical experience with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings) is required. Medicare Home Health and OASIS documentation experience highly preferred.

As a home care Registered Nurse, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Registered Nurse (RN) Responsibilities:

  • Follow a designated care plan in accordance with patient's needs
  • Make home visits to clients in designated geographic territories
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments as necessary
  • Case management and coordination
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

  • A current license as a Registered Nurse in Nevada.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
