Position Overview

The Weekend Admissions Registered Nurse is responsible for completing home health admissions, conducting comprehensive patient assessments, initiating plans of care, and coordinating services to ensure patients receive high-quality care from the start of their home health journey. This position serves as the primary admissions nurse Friday through Monday.

Key Responsibilities

• Complete home health admissions and Start of Care (SOC) assessments in accordance with physician orders and agency policies
• Obtain and document comprehensive medical histories
• Perform physical assessments, including vital signs, mobility evaluations, and safety assessments
• Complete OASIS documentation accurately and within required timeframes
• Develop individualized plans of care based on patient needs and physician orders
• Educate patients and caregivers regarding diagnoses, medications, treatment plans, and home health services
• Coordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team members
• Ensure compliance with all federal, state, and agency regulations, including HIPAA requirements
• Identify and communicate changes in patient condition to the appropriate healthcare providers
• Facilitate referrals for additional home health services as needed
• Maintain timely and accurate clinical documentation

Qualifications

• Current and active Registered Nurse (RN) license in the State of Ohio
• Minimum of one year of nursing experience required; home health experience preferred
• OASIS documentation experience strongly preferred
• Experience with WellSky (Kinnser) preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver's license, reliable transportation, and automobile insurance
• Current TB test and physical examination in accordance with agency requirements
• Ability to attend orientation and training at the Aspire Home Healthcare of Ohio office
