  • Hours: full time .9 FTE/36 hours per week
  • Work Schedule: day/overnight rotation, every third weekend
  • Unit Description: the Transitional Care Unit at Emplify Health by Bellin is a specialized 4-bed unit dedicated to providing comprehensive care for patients transitioning from acute hospital setting to home or other care facilities. Our multidisciplinary team delivers personalized, patient-centered care aimed at optimizing recovery, enhancing functional independence and ensuring a safe and seamless transition to the next level of care.

Job Description:
  • The Registered Nurse (RN) is responsible and accountable for providing quality professional nursing care to patients and their families and significant others adhering to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing, and/or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses and Bellin Health Policies and Procedures. The responsibility includes providing a safe environment for patients, families, staff, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for the patient. Community involvement and collaboration is encouraged.
