Virtua has introduced a new and exciting form of Nursing - "Hospital from Home" (H@H). The Hospital at Home program is a technology driven care model that allows our patients to receive med/surg level of care safely in their home. This can provide you with the personal interaction that you are missing on the floor.
Each patient is selected by a criterion that is set forth by a panel of Physicians, Nurses and Administrators. Patients are identified either during a current admission or directly from the Emergency room. They are very excited to receive care in their home. Hospital at Home allows them to fully recover in their own environment.
H@H requires our nurses to make two visits per day (or more as needed) to assess our patients, deliver medications, provide education and other nursing interventions. Mileage and all necessary equipment will be provided. DME is determined prior to discharge and is sent to the home.

Job Summary:
The Hospital at Home focused Registered Nurse will bring the essential elements of the inpatient care delivery model into the patient's defined living space through the use of leveraged technology and remote clinical care coordination. The Hospital at Home RN provides quality care to patients through the implementation of the nursing process applied through a team nursing model. The Hospital at Home RN will access and document the care of patients and the RN will work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient's home while maintaining high quality and safety standards. The Hospital at Home RN will work in patient homes as well as on-site at the Hospital at Home Command Center and will be expected to work within the surrounding communities of each of Virtua's five acute care hospitals.
Position Responsibilities:
Perform and document nursing process - assessment, diagnosis, planning, implementation, and evaluation of patients receiving care remotely or in-person at their defined home. Review and follow-up on patient education, implemented patient care plans, and coordinated services. Triage, identify, and act on changes in patient condition.
Utilize and instruct patient and family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth visits.
Adheres to policies and procedures of the organization, including HIPAA and Patient confidentially regulations.
Maintains nursing supplies and anticipates and restocks needed supplies. Maintains safe and clean working environment.

Position Qualifications Required:
Required Experience:
Minimum of 2-3 years of acute care nursing experience. Experience working in an emergency department setting or home health preferred. Excellent assessment skills and clinical judgement.
Required Education:
Graduate of an accredited nursing program.
BSN preferred.
Training / Certification / Licensure:
Current State of New Jersey nursing license to practice as a registered nurse. Current CPR certification and BLS.
Must have current proof of valid driver's license and auto insurance.
Hourly Rate: $44.61 - $61.97 The actual salary/rate will vary based on applicant's experience as well as internal equity and alignment with market data.
