PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Columbia, South Carolina.

Position Overview
We are seeking a skilled and autonomous Registered Nurse (RN) to provide home health admissions and case management services throughout Richland County. This role includes overseeing LPNs, coordinating patient care, and ensuring high-quality clinical outcomes. The ideal candidate is experienced in OASIS documentation, demonstrates strong time management, and is confident in wound care management.

Schedule & Work Details

  • Location: Richland County
  • Schedule: Monday through Friday, with flexibility for visit scheduling; office hours 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 40 hours
  • Productivity Expectation: 30 points per week
  • On-call/Weekend Requirement: None
  • Pay Rate: $51.00 to $53.00 per hour
  • Mileage Reimbursement: Yes, between patient visits; mileage for the first visit is reimbursed based on distance exceeding commute from home to agency
  • Dress Code: Business casual or scrubs

Key Responsibilities

  • Conduct admissions for home health services, including comprehensive OASIS assessments
  • Provide ongoing case management for a designated patient population
  • Oversee and support LPN staff, ensuring quality and compliance in patient care delivery
  • Perform skilled nursing visits and patient evaluations
  • Develop, implement, and update individualized patient care plans
