Job description

The Transitional Care Charge Nurse is the dedicated resource that supports, plans, leads and evaluates direct patient care activities during assigned shift. This position provides and oversees skilled nursing care in accordance with the Nurse Practice Act and requires staff accountability for compliance with regulatory and professional standards for patient care and staff.

Percent of Time
Supervises and supports nursing staff in provision of outstanding patient care.

Performs skilled nursing care in accordance with Nurse Practice Act.

Performs administrative duties such as recordkeeping, multi-disciplinary communication, scheduling and other necessary tasks to ensure daily operations are complete.

Coach and assist nursing staff with clinical instructions, mentor and guide as needed.

Serve as liaison with patients' families and patient care team to ensure quality.

Other duties as assigned.

Education:
Associates RN, Bachelor's Degree preferred in Nursing. Ability to speak, understand, read, and write English required.
Experience:
1+ year as RN Staff preferred
License/Certification/Credential:
Registration and current RN license in Oklahoma. CPR and PALS Certification.
