The Home Health Nurse plays a vital role in delivering comprehensive patient care within a small team environment, typically consisting of 2-5 nurses. Reporting directly to the Nurse Manager, this position involves conducting patient assessments, coordinating care, managing medications, and educating patients primarily in chronic disease management and post-surgical care. The role requires regular local travel and on-call availability to ensure continuous patient support and optimal health outcomes.
