Job description

We are seeking a compassionate, dependable, and motivated Registered Nurse (RN) to join our Home Health team. This is a full-time position serving patients in Corpus Christi, Beeville, Mathis, Sinton, and surrounding communities.


As a Home Health RN, you will provide skilled nursing care to patients in the comfort of their homes while helping them maintain independence, manage their health conditions, and improve their overall quality of life. This position is ideal for an RN who is organized, patient-focused, and comfortable working independently in the field.

Our agency also uses NestMed AI to help support clinicians with completing documentation and visit notes more efficiently, allowing nurses to spend more time focusing on patient care.


Responsibilities:

  • Complete comprehensive skilled nursing assessments in the patient’s home
  • Develop, update, and follow individualized plans of care
  • Provide skilled nursing visits in accordance with physician orders and home health regulations
  • Administer medications and treatments as ordered
  • Monitor vital signs and assess changes in patient condition
  • Provide wound care, dressing changes, disease process teaching, medication education, and patient/caregiver instruction
  • Perform venipuncture, lab draws, and IV-related services as needed and within scope of practice
  • Complete OASIS assessments and required home health documentation accurately and timely
  • Coordinate care with physicians, therapists, aides, office staff, and other members of the interdisciplinary team
  • Communicate patient changes, concerns, and needs promptly to the appropriate team members
  • Promote patient safety, compliance, and quality outcomes


Requirements:

  • Current and valid Registered Nurse (RN) license in the State of Texas
  • Minimum of 1 year of nursing experience preferred
  • Home health experience strongly preferred
  • Knowledge of OASIS, skilled nursing documentation, and home health regulations preferred
  • Strong assessment, communication, and critical-thinking skills
  • Ability to work independently and manage a field schedule
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to travel throughout Corpus Christi, Beeville, Mathis, Sinton, and surrounding service areas
  • Must be dependable, professional, and committed to quality patient care
