Job description

American Traveler is hiring a Home Health RN with required Homecare Homebase (HCHB) experience for a 13-week contract in a home health setting serving an adult patient population.
Job Details
• Home health care setting serving adult patients only,
• Caseload of approximately 30 patients with a daily point target of 6.5,
• Responsibilities include start of care (SOC), recertifications, and daily visits,
• Schedule is Monday-Friday, 8am-5pm with a weekend rotation,
• No on-call requirements,
• A phone and tablet will be provided for use in the field,
• Charting is performed using Homecare Homebase (HCHB),
Job Requirements
• Active KY RN license or Compact (NLC) RN license - must be active at time of consideration; pending licenses will not be accepted,
• Current BLS certification (AHA) required; pending certifications not accepted,
• Minimum 2 years of RN experience required,
• At least 1 year of Med/Surg experience in a hospital setting required,
• Home health experience required - no exceptions,
• Homecare Homebase (HCHB) EMR experience required - no exceptions,
• A copy of a valid driver's license is required for consideration,
• Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire,
Additional Information
• Orientation consists of approximately 2 weeks, including field visits with a dedicated work coach (approximately 12 orientation hours total),
• Clinical support is available 24/7 for emergencies and during business hours (8am-5pm) via a clinical manager or educator,
• Holiday rotation coverage is required,
• Scrubs are required; no color restriction,
• BSN is not required for this position
