Registered Nurse in West Springfield, MA | Baystate Health Careers

Baystate Health is seeking a Registered Nurse to join our Home Health Team. Our team provides compassionate, skilled care to patients in the comfort of their own homes, working closely with patients, families, providers, and interdisciplinary care teams to deliver personalized, high-quality care and support positive health outcomes.

The Home Health RN role offers the opportunity to make a meaningful impact through individualized, patient-centered care while supporting patients and families in their homes and collaborating with a dedicated interdisciplinary team.

The Registered Nurse Will:
  • Provide skilled, compassionate nursing care to patients in their homes
  • Assess patient needs, develop individualized care plans, and monitor progress.
  • Collaborate with physicians, APPs, therapists, social workers, and other members of the care team.
  • Identify changes in patient condition and coordinate timely interventions or escalation of care.
  • Promote patient independence, safety, and quality of life in the home setting.
  • Complete accurate clinical documentation and communicate patient updates with the interdisciplinary team
  • Build meaningful relationships with patients and families while supporting them through their healthcare journey.
Schedule Details

  • Hours: 40 hours
  • Schedule: 8a-4:30p (with some flexibility)
  • Weekend Requirement: One weekend a month (weekend differential)
