This position is remote, allowing you the flexibility to work from the comfort of your own home while still being a vital part of a supportive professional community. Our team operates in a cohesive and collaborative manner, and while the majority of your work will be conducted remotely, you will have periodic opportunities to connect with colleagues during team meetings and training sessions. You will be equipped with all necessary tools and technologies, including telehealth platforms and electronic medical record systems, to ensure seamless service delivery.

• Conduct detailed mental health assessments and evaluations to accurately identify individual clients’ needs, utilizing diagnostic tools and methodologies.
• Create personalized treatment plans that integrate evidence-based practices, tailoring therapeutic approaches such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and mindfulness techniques, among others.
• Engage in therapeutic sessions with clients, providing enriching therapeutic experiences that address their emotional, psychological, and social needs.
• Collaborate effectively with interdisciplinary team members, including case managers and psychiatrists, to coordinate holistic care approaches that address both mental and physical health considerations.
• Implement crisis intervention strategies with compassion and expertise, guiding clients through acute mental health emergencies and facilitating appropriate resources.
• Participate in community outreach initiatives designed to raise mental health awareness, reduce stigma, and educate the public about available services.

Your work will primarily focus on adults and families facing mental health challenges and end-of-life issues in a hospice setting. Clients may experience complex emotional responses, such as grief, anxiety, and depression. You will provide empathetic support, helping them navigate their feelings and foster coping strategies tailored to their unique situations.

• Must possess an active, independent license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
• A minimum of a Master’s degree in Social Work, Psychology, Mental Health Counseling, or a related field is required.
• Exceptional communication skills, with a proven ability to build strong therapeutic alliances with clients from diverse backgrounds.
• Cultural competence and sensitivity in working with individuals belonging to varied ethnic, cultural, and socioeconomic groups.
• Strong organizational abilities, ensuring timely and comprehensive documentation of client interactions and treatment plans.

• Experience in hospice or palliative care settings is highly valued, along with familiarity with the unique emotional and psychological challenges faced by patients and families during end-of-life care.
• Demonstrated ability in utilizing telehealth methodologies effectively to engage and support clients virtually.
• Experience with community outreach and public speaking aimed at mental health education.
