Are you passionate about supporting the Children & YouthPopulation and making a real difference in their lives? EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 120 impactful programs across Long Island and NYC, is looking for a dedicated full time Case Manager to join the Safe Harbour Program.
  • Salary: $28/ hour
  • Schedule: 9AM-5PM, Monday- Friday with flexibility to work early evenings
  • Location: 84 Wheeler Road in Central Islip, NY and 940 Roanoke Avenue, Riverhead NY.

What drives the core purpose of this job:
The Safe Harbour Case Manager is responsible to provide short and long term, trauma informed, case management and direct services for minor victims of human trafficking and youth at-risk of sex and labor trafficking that are referred to Suffolk County Safe Harbour. The Safe Harbour Case Manager and will coordinate with additional human services agencies as needed.
What are the key Responsibilities:
  • Complete risk-assessments for minor victims of human trafficking and youth at-risk of human trafficking that are referred to Safe Harbour
  • Provide trauma informed, victim centered case management services and outreach to minor victims of sex and labor trafficking and youth at-risk of sex and labor trafficking to youth in Suffolk County, NY.
  • Conduct assessments for client needs and develop an individualized service plan to support clients with safety planning, goal setting and psychoeducation.
  • Ensure service plans and referrals address the safety, security and healing of youth served in the program, and are developmentally appropriate.
  • Document in the CAC NYSCA database the services provided to the client to ensure accurate case records are maintained.
  • Administer and document iMPRoVE case closure surveys.
  • Collaborate with community partner agencies to deliver staff training regarding the identification of vulnerable minors at risk of sex and labor trafficking and connect them to services.
  • Transport and escort clients to court proceedings, medical appointments, and other appointments as needed.
  • Assist with applying for public benefits, employment, medical care, return of personal property and short- and long-term coordination of such services.
  • Make appropriate referrals for medical exams and mental health services
  • Maintain an accurate database of professional referral resources that assist the stated population.
  • Prepare monthly and quarterly statistical and annual reports as required by the funding source.
  • Attend workshops and training as needed for project implementation
  • Assist in preparing reports for the grant as needed.
  • Participate in team meetings, conferences, agency presentations, trainings and related events.

What key knowledge, skills, and abilities are required to excel in this role:
  • Bachelor's Degree is required, a degree in Mental Health Counseling, Social Work, Forensic Psychology or Behavioral Health field preferred.
  • Minimum of two years of relevant experience
  • Must be proficient in computer applications or other automated systems such as Google Drive, spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  • Excellent communication, organizational, and interpersonal skills.
  • Must have access to automobile and possess valid NYS driver's license.
