Transitions At Home, is currently seeking to hire a Part-Time CNA/Home Health Aide in the Spooner area. We’re hiring a dependable and compassionate CNA/HHA to provide hands-on personal care and support to patients in their homes. What does a Transitions at Home CNA/HHA do?
Transitions At Home CNA/Home Health Aide provides personal cares, bath and hands on care, household services. Reports change in condition and needs to the RN Case Manager in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
