Job description

Capitol View Transitional Care Center is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital (we are part of the HealthPartners organization as a separate nonprofit organization.) We provide quality care to patients from 18+ years of age who are in need of a short-term stay. Our average length of stay is 17 days. As a RN/LPN your patient to nursing staff ratio is three times lower than the average nursing home. In this role you will be responsible for the following job duties:
Provide direct care to patients by providing ADLs, accessing physical and mental status of patient to determine efficacy of treatment and care plan, administer medication complete all documentation, etc.
Prepare room preparation and room discharge of patients
Answer call lights and provide follow-up to ensure needs are met.
Answer questions from patients and families or direct them to the appropriate person.
Performs other duties as assigned.
 
FTE: 0.0; Per Diem/Casual
 
Work Schedule: Required to work a minimum of 4 shifts per month - 2 shifts must be a weekend shift.

LPN starting rate at $28.29/hour. Pay rate is based off previous LPN work experience.
RN starting rate at $36.47/hour. Pay rate is based off previous RN work experience.
 
Required Qualifications:
Graduate of an accredited school of nursing
Currently licensed as a LPN or RN with the Minnesota Board of Nursing
Current CPR Card Certification
Must be able to attend and have reliable transportation to and from HealthPartners' Nursing Orientation in Bloomington on Tuesday and Wednesday from 8am-430pm.
