Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team at MemorialCare Health System. In this full-time remote position, you will play a crucial role in providing essential psychosocial support for patients and their families within our Hospice Center.   • Provide crisis intervention and support to patients and their families, facilitating immediate solutions to urgent issues.
  • Coordinate with healthcare providers, ensuring that psychosocial and socioeconomic needs are systematically integrated into care protocols and discharge plans.
  • Deliver ongoing education to your healthcare team about the critical role of social work in patient care and the psychosocial factors that influence treatment outcomes.

Qualifications
  • A Bachelor’s Degree in Social Work from an accredited institution.
  • Current LCSW licensure in California.
