This role offers the flexibility of remote work while primarily serving clients in the Springfield area. As part of our Hospice Center, you will engage with individuals and families during some of the most challenging times in their lives. • Provide direct mental health services to clients, working both in the office and remotely, to assess and treat symptoms of major mental illnesses and emotional disorders.
• Conduct brief, evidence-based screenings and, when necessary, arrange for comprehensive assessments to inform treatment planning.
• Collaborate closely with inter-professional teams to develop and implement integrated care plans that ensure clients receive appropriate services.
• Maintain accurate and timely documentation of client progress, treatment plans, and case management efforts, including adhering to ethical guidelines and organizational standards.
• Interact with clients from diverse backgrounds, demonstrating respect and understanding of their unique cultural contexts and needs.
• Participate in crisis intervention efforts and develop safety protocols for high-risk clients.
• Carry a caseload that includes individual, group, and family therapy sessions tailored to client needs.
• Monitor and evaluate treatment effectiveness through regular client meetings, ensuring timely adjustments to care as required.
• Engage in quality improvement initiatives and committee work to enhance service delivery and operational efficiency.

You will work specifically with clients suffering from severe mental health issues and complex emotional disorders at our Hospice Center, emphasizing the need for empathy and strategic intervention. Our clientele may range from individuals coping with chronic mental illness to those facing acute emotional crises, particularly in the context of end-of-life care.

• A Bachelor’s Degree in Social Work from an accredited institution, with an active LCSW license.
• A minimum of two years of direct clinical experience in mental health care, preferably in hospice or similar settings.
• Demonstrated capability in crisis management, assessment, and diagnosis, alongside a solid understanding of treatment modalities.
• Exceptional communication skills, both verbal and written, with the ability to build rapport with clients and collaborate with colleagues effectively.
• Strong organizational skills and attention to detail to manage caseloads and ensure compliance with documentation standards.

• Experience working in hospice or palliative care environments.
• Familiarity with electronic health record systems to streamline documentation and communication.
• Proven ability to engage in community outreach and promote mental health services in under-resourced populations.
