What You'll Do
  • Evaluate patient functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.
  • Evaluate home environment for hazards or barriers to more independent living
  • Identify equipment needs
  • Provide supervision to the Occupational Therapy Assistants
  • Conduct an initial evaluation/assessment and report findings to the Health Care Provider and RN and develops the plan of care promptly for patients
  • Document initial evaluation and treatment plan and re-evaluations of patient's progress at least every thirty (30) days and update plan of care as necessary.
  • Complete comprehensive assessments and OASIS data, as indicated on a timely basis.
  • Observe, record and report to Health Care Provider the patient's treatment, response and progress and promptly report significant changes
  • Obtain orders to revise plan of care
  • Instruct patient/CG in the use of alternate methods and devices for upgrading independence in activities of daily living

Qualifications
  • Bachelor's degree and active Occupational Therapist (OT) license in the state of California
  • Minimum of one (1) year of clinical experience under a licensed Occupational Therapist; home health experience preferred
  • Ability to perform in-home occupational therapy evaluations and treatments
  • Strong documentation skills with the ability to complete and submit records in a timely manner
  • Experience with Home Care Home Base or other home health EMR systems preferred
  • Must have a reliable vehicle, valid driver's license, and current automobile insurance
