Job description

Medical Scheduler - Hybrid or Remote
Position Summary:
Eventus WholeHealth delivers an integrated model that provides holistic care for medically vulnerable adults achieving extraordinary outcomes for the people we serve. We do this through a value driven framework of excellence, integrity, person-centeredness, stewardship and teamwork. This care is provided to adults who reside in skilled nursing, assisted living facilities and private residences through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Worker, Clinical and Administrative Support Staff.
In this role, you'll communicate effectively with patients, facilities, medical assistants, care team coordinators and providers to ensure the patients' needs are being met.
Essential Duties/Responsibilities:
  • Coordinate facility and patient level scheduling for all providers in each of the assigned facilities / territory
  • Ensure Care Teams have scheduled routine visits and pain visits, and/or as needed, medication visits, or any additional types of visits that have been deemed a medical necessity
  • Ensure that each patient is seen as medically necessary, per clinical protocol
  • Ensure scheduling rosters are updated daily to ensure accuracy
  • Ensure patient rosters are appropriately managed and acute visits and day of visit schedule changes occur timely
  • Assist Care Teams to adjust as needed for PTO, sick days, or unusually high volume of new patient referrals
  • Assist with reporting reasons for missed visits or provider concerns to Care Team Clinical Manager for provider management
  • Coordinate scheduling/patient related communication with the new patient registration department and care teams
  • Assist with planning and evaluating patient registration methods with patient registration team
  • Ensure that facilities or patients receive copies of all scheduled patient visits and assist with problem solving any issues
  • Ensure any appropriate CCM services are documented.
  • Notify Care Team Support Managers and Care Team Clinical Managers of any outstanding open notes by providers
  • Work with Customer Account Managers to assist with resolving scheduling related facility issues/concerns
  • Check all forms of communication several times daily to ensure patient needs are met
  • Ensure communication regarding transitions of care to CCM team to enhance patient care and education
  • Assist with calls from facilities and patients regarding questions, issues, or concerns that cannot be triaged by a Care Team Advocate or Coordinator
  • This is a hybrid position. Candidates must live in or near our Chapel Hill office.

Education:
  • High School Diploma

Skills and Qualifications:
  • 2 years of telephone or customer service experience required
  • 1 year of prior healthcare scheduling or medical related experience required
  • Experience with an Electronic Medical Record (EMR) and/or practice management system
  • Familiarity with medical terminology required and knowledge of HIPAA privacy laws
  • Excellent PC skills required; strong excel skills preferred
  • Competent knowledge of Microsoft office; strong Excel skills required
  • Ability to work under pressure of tight deadlines and complete task accurately
  • Ability to effectively handle multiple priorities with detail and accuracy
