We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at William Penn Nursing and Rehab, a 121-bed skilled nursing facility located in Lewistown, Pennsylvania, within walking distance to The Geisinger – Lewistown Hospital. As a Registered Nurse (RN) at our facility, you will have the opportunity to make a meaningful impact on the lives of our residents and their families.

About the Role:

As a Registered Nurse (RN), you will provide high-quality, patient-centered care to our residents, including those with complex medical needs. Your duties will include:

  • Assessing and planning individualized care for each resident

  • Administering medications and treatments, as prescribed by physicians

  • Developing and implementing individualized care plans

  • Collaborating with other healthcare professionals to ensure comprehensive care

  • Educating residents and their families on health and wellness

Responsibilities:

  • Provide direct patient care to residents, including but not limited to:

    • Conducting rounds and assessing resident conditions

    • Administering medications and treatments

    • Developing and implementing individualized care plans

    • Monitoring and reporting resident conditions to physicians and other healthcare professionals

  • Collaborate with other healthcare professionals to ensure comprehensive care, including but not limited to:

    • Physician and nurse practitioner consultations

    • Social worker and rehabilitation therapy consultations

    • Dietary and activities department consultations

  • Participate in quality improvement initiatives to improve patient outcomes and satisfaction

  • Maintain accurate and up-to-date records of patient care

  • Perform other duties as assigned by the Director of Nursing
