We are thrilled to announce that Champion City Nursing & Rehabilitation, a premier healthcare organization, is now under new ownership! As we embark on this exciting new chapter, we are seeking a dedicated and compassionate MDS Nurse to join our team.

About the Role:

As a MDS Nurse, you will play a vital role in creating and implementing individualized care plans for our residents, ensuring compliance with regulations, and delivering exceptional care with a focus on resident-centered services. You will collaborate with the interdisciplinary team to develop and implement comprehensive care plans, conduct thorough assessments and evaluations, and ensure compliance with federal, state, and local regulations, including MDS guidelines and regulations.

Responsibilities:
  • Collaborate with the interdisciplinary team to develop and implement comprehensive care plans for residents
  • Conduct thorough assessments and evaluations to identify resident needs and preferences

Requirements:
