The Physical Therapist will work onsite in our warm and inviting rehabilitation facility, equipped with state-of-the-art technology and resources. Your primary interactions will revolve around a diverse patient population, ranging from post-operative individuals to those managing chronic conditions.

Key Responsibilities

As a Physical Therapist at Pinnacle Rehab Associates, your responsibilities will include but are not limited to:

  • Directing personalized physical therapy treatments focused on preventing disability, relieving pain, and enhancing physical function following the established plan of care.
  • Overseeing the implementation of treatment plans by rehabilitation assistants, ensuring adherence to quality care standards.
  • Conducting thorough evaluations of patient functional levels through diagnostic tests and assessments tailored to individual capabilities.
  • Assessing home environments to recommend modifications that enhance rehabilitation and support patient independence.
  • Providing prescribed physical therapy services in the home, including therapeutic exercises, traction, and massage therapy.
  • Documenting clinical records accurately, including evaluations, progress notes, and discharge summaries in compliance with regulations and to guide team members.
  • Collaborating with an interdisciplinary team during case conferences to develop and refine comprehensive care plans.
  • Training patients and their families on physical therapy devices and ensuring they understand their treatment regimen.
  • Staying accessible to physical therapy assistants during the administration of treatments to ensure ongoing support and guidance.
Patient Population / Client Focus

The Physical Therapist will serve a diverse clientele, encompassing a variety of rehabilitation needs such as post-surgical recovery, sports injuries, neurological disorders, and orthopedic conditions. Our approach emphasizes patient-centered care, integrating family input into the treatment process to ensure optimal outcomes and a holistic approach to recovery.

Qualifications

The ideal candidate will possess the following qualifications:

  • A Bachelor’s Degree from an accredited physical therapy program.
  • A valid and current PT license in the state of Texas, in good standing.
  • One year of relevant experience in an acute care or home care setting.
  • Current CPR certification to ensure the safety of all patients.
  • Valid driver’s license and automobile insurance for home visits.
  • A creative mindset with strong decision-making abilities.
  • A passion for clinical excellence and patient-centered care.
Preferred Experience

While one year of experience is required, candidates with prior experience in telehealth practices, advanced orthopedic techniques, or leadership roles in physical therapy settings will be highly regarded. Familiarity with electronic health records (EHR) systems and clinical documentation standards is preferred to streamline patient management and care coordination.

Compensation and Benefits
